George, 1/25 11:49 comment very well stated? We should all print and save and reread from time to time. Maybe add, ....poor oral hygiene AND POOR innate natural defense mechanisms. Also ... Overloaded or under -engineered prostheses and support structures. Good work and Best wishes, Paul Kozy

Dr. Vinicio Pedrazzoli, I agree that a fixed appliance may create oral hygiene program difficulties with the patient. However, regardless of what appliance is placed-primary bar for overdenture or fixed appliance, the patient should clean it. Many of these patients lost their natural dentition to plaque induced dental diseases due to poor oral hygiene that despite their personal efforts or professional guidance. In my opinion, the patient's oral hygiene behavior or personal skills usually do not change significantly when the receive an implant based solution regardless of the prosthetic solution or the amount of investment they make in their future oral health. Dr. Cristiano Pereira Alves In the program, I mentioned a recent textbook by Testorini on Immediate Loading. In their book, they used 3 methods of constructing the intraoral verification jigs(IVJ) and then examined the fit of the final bar. I used for this seminar two of the methods for the fabrication of the IVJ for my own internal study the Testorini, etal used. My clinical observation of fit of bars is the same as Testorini--the bars fit well. My preferred method is to laboratory fabricate bar with GC Patter Resin, section with thin disc, have custom tray, lute with GC pattern in mouth, polyether impression with custom tray, box impression, and pour immediately with Tallidium grey scanning die stone. This becomes master cast. We use Procera CAD-CAM technology to fabricate the bars--they are guaranteed to fit. I have found that this protocol works well in my hands. Dr. Duello
