解説:
Dr. Georgios Romanos, Professor of Clinical Dentistry at the Eastman Dental Center, University of Rochester, presents the research and clinical data that has prompted the successful utilization of immediate implant placement and immediate loading in a variety of settings. A discussion of factors such as implant design, primary stability, rigid connection and abutment design are highlighted in their role in optimizing clinical success.

Danny;
Welcome to the blog. A really good CE course that reviews CBCT in clinical practice is on this site by Dr. Maurice Salama.
Take a look, it is well done and gives a nice review of how it can be used for surgery and implants.
Good luck

This is my 1st video after joining Dental XP as a premium member. Very exciting presentation about what is possible. How do you use cone beam volumetric imaging?

Georgios Romanos - (2010/01/17 14:05)
Thank you for your interest in my lecture. I hope you enjoyed the presentation.
The studies of Dr. Lindhe are related to results of animal (dog) studies, where the measurements have been performed after tooth extractions. We expect to have always some bone resorption in the height and the width, which is related to different factors such as bone remodeling, blood supply, elevation of a flap or not, augmentations or not etc. In the presented concept, placing the implants deeper and without removal of the abutments, we have crestal bone stability in case of platform switching. We have used this concept for years and published the first animal and human results.
The reality shows today that I am able to have similar results with different implant systems if we have similar requirements.

I enjoyed the presentation and especially appreciated the summation at the end of all of the considerations that one should take with immediate implant placement. My question for Dr. Romanos is this; Could you clearly define your rational for subcrestal placement. Is it predicated on our understanding from Dr Lindhe's article that we will see at least a 1mm loss of buccal plate or is it a combination of this aforementioned fact and the implant and prosthetic design specific to the Ankylos implant.

very well coordinated program by one of the experts in this field, that shows that emphasizes the different aspects to take into account in this incredibly valuable, increasingly predictable approach
Excellent documentation and presentation George. . .

Dr. Romanos performs a passionate and clinically relevant presentation on the current standard of Immediate Implant Placement and Loading. A concept of treatment that still remains with some controversy. How many on this blog perform these kind of full arch immediate replacement cases with immediate loading? We do here in Atlanta and have had tremendous success since the mid 1990's and have followed these cases for more than a decade with excellent results. No prosthesis has ever failed and our implant success rivals conventional placement methods.

Controversies Surrounding Delayed vs. Immediate Implant Placement: Is There No Middle Ground?
This presentation looks critically at the literature and attempts to give clinicians a more flexible approach to immediate and delayed implant placement. Critical surgical guidelines and clinical pearls will be offered for both types of treatment as well as the management of some of the complications that may arise from immediate or delayed implant placement.

Soft Tissue Recession Around Implants
When treatment with dental implants is indicated, an accurate diagnosis must be made to evaluate the clinical parameters and determine the optimal time for immediate or delayed (ie, early or late) implant placement and loading following tooth extraction. It is also important to identify complications and their implications on the aesthetic outcome.1 This article explains behavior of the hard and soft tissue around the implant, evaluates the timing of implant placement after extraction, and reviews various parameters that influence tissue marginal remodeling.

Computer-Generated Abutment Using A Coded Healing Abutment: A Two-Year Preliminary Report
This article presents a preliminary clinical evaluation following the placement of 107 dental implants covered with computer-coded healing abutments. The healing abutment system evaluated herein provided information to specific computer software that facilitated the delivery of an anatomical definitive abutment. This system also assisted in the development of a titanium abutment whose characteristics mimicked the natural contours of the teeth being replaced. A two-year postoperative evaluation was performed to evaluate occlusion, gingival integrity, contact areas, inflammation, food impaction, porcelain integrity, radiographic condition and overall patient satisfaction. Improved tissue response and resultant aesthetics were observed.
