解説:
Through maximum usage of bleaching, bonding, laminating and crowning, the dentist can alter virtually all discoloration aspects of the smile, ranging from mild to severe. However, knowing which treatment to use, when and where, is still a question of judgment. This course will highlight diagnosis and treatment of some of the major esthetic problems encountered including, treatment planning and step-by-step procedures to accomplish the most in the least amount of time. Dr. Goldstein will also present a new comprehensive approach to management of patients with discolored teeth.

Cyndi,
If the stains are in the incisal thirds, depending on the color of the stain, you might consider enamel micro-abrasion...certainly the white hypocalcification stains can many times be successfully treated this way. Otherwise I would consider preparing the tooth with an AC10 football shaped diamond(BrasselerUSA) to take as much of the stain out and then bond either using an opaquer first or an opaque composite such as one of the opaque Venus Diamond (Heraeus) shades that are quite good at masking stains even without additional opaquer. The MI paste is for desensitization so if the patient needs it fine...otherwise the desensitization from the composite bonding technique will most times take care of any sensitivity.

Thank you for the great tips , what if the stains are only int he incisal thirds of the 2 centrals, would you advise me to use the MI paste or should i go with masking it with composite?

Yes, fluoride can be effective but MI Paste (GC) is more effective. And if you use a bleaching agent that has a desentization agent in it then there should be much less sensitivity.

I have question, How we can overcome the sensitivity problem after the bleaching..Is applying flouride is effective ?? Any other ways?

HI DOC,GREAT PRESENTATION AND GOOD LECTURE....MY QUESTION IS,HOW SENSITIVE ARE TEETH TO THESE BLEACHING TECHNIQUES.POST OP?COZ I AM PRACTISING IN A COLD CLIMATE.?WHAT INSTRUCTIONS DO WE ADVICE REGARDING FOOD AND ANY SPECIAL CARE ?TO AVOID DISCOLORATION.

Hi Joao
we find that Ultradent Opalescence Endo works very well as an internal Bleach product ."walking Bleach" - - it is a 35% HYDROGEN PEROXIDE as compared to carbamide peroxide , and so is effectively much stronger - - it is also Catalysed by adding a Heat source such as a bleaching wand
- - there is little research data to indicate that light/heat accelerates traditional bleaching but clinically it does look brighter/ lighter possibly because of dessication. . .also may induce a little more sensitivity
anyone else have similar experience ?

Hi Dr Goldstein! I tried all kind of products to bleech and i can say that opalescense always got funtastic. After that i recommend a toothpaste that helps the remineralization as nutri-émail does very well. Whenever i use light bleeching i iam not so happy. What is the best product to do endo bleech? thanks for your lecture. hug
