天然歯の長期的安定のマネジメントにおける結合組織の重要性

解説:
In factoring in all the protection biologic tissues surrounding the tooth no greater tissue, attached connective tissue serves a more important role in the long term maintenance of natural teeth. By the nature of connective tissue (minimal vascularity and dense) it has built in properties and really acts as the main barrier between bacteria and the underlying Periodontium. One key factor is is must be attached or is serves as no protection. The video presented will show clinical long term evidence of its importance along with different techniques to achieve such results. Critical to ones understanding it must be accepted that Recession means only one thing- BONE LOSS.

Sam, 100% of the time I would prefer palatal tissue over Perioderm. No vestibule is a real killer for either Alloderm or Perioderm. All mucosal attachment. I amvery happy with Perioderm but the palate is my favorite and will always be. Sorry for the delay. I did not know there were any questions. Thanks, Danny

Paul, There may be a misunderstanding. I use Perioderm all the time. I no longer use Alloderm as all as Perioderm is so much easier to use. Perioderm is: 1. Same consistency for the entire piece of material 2. Easy to suture 3. When moist very easy to contour 4. Lays on the teeth not on your gloves. Every friend I know that switched to Perioderm has thanked me.

Excellent lecture Dr Melker. My compliments to your work and ease to explain.

Nice cases. Just curious how you say that you do not use Perio Derm or Allo derm and then you showed a case that you did use Perio derm with good success. Did I miss something in the discussion?

My compliments Dr. Melker. When would you NOT utilize ACDM tissue over autogenous CTG? thanks Sam

Aesthetic Crown Lengthening

Aesthetic Crown Lengthening
The rationale for crown lengthening procedures has progressively become more aesthetic-driven due to the increasing popularity of smile enhancement therapy. Although the biologic requirements are similar to the functionally oriented exposure of sound tooth structure, aesthetic expectations require an increased emphasis on the appropriate diagnosis of the hard and soft tissue relationships, as well as the definitive restorative parameters to be achieved. The development of a clinically relevant aesthetic blueprint and attendant surgical guide is of paramount importance for the achievement of successful outcomes.

Soft Tissue Management and Functional Mock-Ups
Patients desiring a cosmetic smile improvement can present treatment planning challenges. An excessive display of gingiva resulting from a high lip line often complicates treatment planning and can compromise the final esthetic result. As we evaluate the patient, we often have to ask ourselves, do we “lower the river” (increase tooth length incisally), “raise the bridge” (increase tooth length gingivally) or use a combination of both approaches? This article addresses this issue using a methodological…
